Chronic diseases pose a significant health challenge globally and effective management is essential to improve patient outcomes and reduce the burden on healthcare systems.<\/p>\n\n\n\n
Speaking during a meeting with sub-county focal persons, Promotive and Preventive Health Chief Officer Dr. Paul Wangwe noted that the team plays a crucial role in supervising and coordinating care for patients with chronic conditions.<\/p>\n\n\n\n
He said that these focal persons are equipped with adequate reporting tools that enable them to accurately record and monitor key statistics related to chronic disease cases within their designated sub-counties.<\/p>\n\n\n\n
\n“The availability of these reporting tools allows sub-county focal persons to maintain detailed and up-to-date information on the prevalence of chronic diseases, the effectiveness of management plans, and the outcomes of interventions,” said Dr. Wangwe.<\/strong><\/p>\n<\/blockquote>\n\n\n\n
He added that by collecting and analyzing the data, they can identify trends, gaps in care, and areas for improvement, ultimately leading to more targeted and efficient healthcare services.<\/p>\n\n\n\n
Directors Dr. Evans Kiprotich and Dr. Sarah Kipkulei noted that one of the key benefits of having precise reporting tools at their disposal is the ability to facilitate well-informed decision-making processes, therefore, the Sub County focal persons can collaborate with healthcare providers, policymakers, and other stakeholders to develop strategies that address the specific needs of patients with chronic diseases.<\/p>\n\n\n\n
